8.2.2 Ascendants, Article 9 (Presidential Decree 106/07) / Initial Issuance or Change of Residence Purpose

      1. Certified photocopy of a valid passport or other travel document recognized by our country and/or, in the case of a previous residence permit, certified photocopies of all pages of the passports covering the period of its validity.
      2. Type C entry visa / entry stamp or residence permit of an EU member state.
      3. Printout of insurance capacity (direct – indirect) issued by a public insurance agency (EFKA – National Social Security Entity) with the submission of: A.M.K.A. – Social Security Number, Α.Μ.Α. – Social Security Member Registration Number, A.F.M. – Taxpayer Identification Number
        OR Insurance policy by a private insurance company.
      4. Certified copy of the Certificate of Registration or the Certificate of Permanent Residence of the EU citizen.
      5. Copy of the marriage or civil partnership certificate or equivalent document proving the family relationship, issued by the authority provided for in the national legislation of the respective country, officially certified and translated(where required), or a marriage or civil partnership certificate issued by a Greek public authority in the case of a marriage performed in Greece.
      6. (a) If the child is an EU citizen: Birth certificate of the EU citizen or Certificate of family status in a foreign language or other official document issued by a local or foreign authority, officially certified and translated(where required), proving the family relationship.(b) If the spouse is an EU citizen: Recent family status certificate or/and birth certificate issued by foreign authorities, officially certified and translated (where required), proving the family relationship.
      7. Documented evidence of material dependence on the EU citizen or the spouse/partner in Greece or abroad, along with a solemn declaration by the sponsor stating that the support is ongoing. The proof may take the form of a document issued by a competent authority in the country of origin or habitual residence certifying that the family member in question is being or has been dependent on the EU citizen or the spouse/partner or living under his/her roof in that country or that there are serious health reasons requiring their personal care by the EU citizen or the spouse/partner. The evidence may take the form of: a) A document issued by the competent authority of the country of origin or habitual residence, certifying that the family member is supported by the EU citizen or the spouse/partner or resided under the same roof in that country b) Medical certificates, certifying serious health reasons requiring their personal care by the EU citizen or the spouse/partner, c) Bank transfers, d) Receipts of rent paid by the EU citizen or the third-country national, e) Tax declaration of the supported member showing no individual income, etc.
      8. Documents required for the issuance of the card:
        • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format
        • Card fee of 16 Euros covering the printing of the separate document (electronic residence permit) (e-Paravolo / fee code 2119).
        • Official public document from the country of birth or origin, officially certified and translated, clearly stating the place of birth. The translation must also indicate the place of birth in Latin characters.(To be provided only if the passport does not show the place of birth in Latin characters)
