The permit is renewed every two years, provided that the same conditions are met.

1. Certifiedphotocopy of a valid passport or other travel document recognized by our country and/or, in the case of a previous residence permit, certified photocopies of all pages of the passports covering its validity period.
2. Type C Entry visa/entry stamp or residence permit issued by an EU member state.
3. Printout of insurance capacity (direct – indirect) issued by a public insurance agency (EFKA – National Social Security Entity) with the submission of: A.M.K.A. – Social Security Number, Α.Μ.Α. – Social Security Member Registration Number, A.F.M. – Taxpayer Identification Number
   or Insurance policy by a private insurance company.
4. Certified copy of the Certificate of Registration or Certificate of Permanent Residence of the EU citizen.
5. Recent Certificate of Family Status or other official document issued by a Greek or foreign authority, officially certified and translated(where required), indicating the family relationship with the EU citizen.
6. Documented evidence of the applicant’s material dependence on the EU citizen or the spouse/partner in Greece or abroad, along with a solemn declaration by the sponsor stating that the support is ongoing. The proof may take the form of a document issued by a competent authority in the country of origin or habitual residence certifying that the family member in question is being or has been dependent on the EU citizen or the spouse/partner or living under his/her roof in that country or that there are serious health reasons requiring their personal care by the EU citizen or the spouse/partner. The evidence may take the form of:
   • A document issued by the competent authority of the country of origin or habitual residence, certifying that the family member is supported by the EU citizen or the spouse/partner or resided under the same roof in that country,
   • Medical certificates, certifying serious health reasonsrequiring their personal care by the EU citizen or the spouse/partner,
   • Bank transfers,
   • Receipts of rent paid by the EU citizen or the third-country national,
   • Tax declaration of the supported family member indicating that they have no individual income, etc.

or Documented evidence of the material dependence of the EU citizen along with a solemn declaration that he/she continues to be supported by the applicant. The evidence may take the form of a document issued by a competent authority in the country of origin or habitual residence, certifying that the family member in question supports the EU citizen or that the EU citizen resided under their roof in that country,

or Recent medical certificate or other official document indicating serious health reasons requiring the personal care of the applicant by the EU citizen.

7. Documents required for the issuance of the card:
   • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format
   • Card fee of 16 Euroscovering the printing of the separate document (electronic residence permit) (e-Paravolo / fee code 2119).
   • Official public document from the country of birth or origin, officially certified and translated, clearly stating the place of birth. The translation must also indicate the place of birth in Latin characters.(To be provided only if the passport does not show the place of birth in Latin characters)
