1. Certified photocopy of a valid passport or other travel document recognized by our country and/or, in the case of a previous residence permit, certified photocopies of all pages of the passports covering the period of its validity.
2. Entry visa type C/entry stamp or residence permit issued by an EU Member State.
3. Printout of insurance capacity (direct – indirect) issued by a public insurance agency (EFKA – National Social Security Entity) with the submission of: A.M.K.A. – Social Security Number, Α.Μ.Α. – Social Security Member Registration Number, A.F.M. – Taxpayer Identification Number
   OR Insurance policy by a private insurance company.
4. Copy of the marriage certificate or equivalent document proving the family relationship, issued by the authority provided for by the national legislation of the country in question (and/or recent Certificate of Family Status of the EU citizen), officially certified and translated(where required) or a marriage certificate issued by a Greek public authority in case of marriage in Greece. (In case of marriage)

or

Copy of the registration certificate of the civil partnership in Greece or equivalent document proving the family relationship, issued the authority provided for by the national legislation of the country in question and recent Certificate of Family Status of the EU citizen and recent Certificate of Family Status of the spouse/partner of the third-country national, officially certified and translated (where required) (In case of civil partnership)

5. Certified copy of the Certificate of Registration or the Certificate of Permanent Residence of the EU citizen.
6. Documents required for the issuance of the card:
   • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format.
   • Card fee of 16 Euros covering the printing of the separate document (electronic residence permit)(e-Paravolo / fee code 2119).
   • Official public document from the country of birth or origin, officially certified and translated, clearly stating the place of birth. The translation must also indicate the place of birth in Latin characters.(To be provided only if the passport does not show the place of birth in Latin characters)

NOTE

In case you hold a Type Application Certificate (Blue Certificate) issued by the Ministry of Migration & Asylum due to the conclusion of a civil partnership, it is not possible to reapply with this legal document for the issuance of a residence permit for the spouse/partner of a Greek citizen under a civil  partnership.
