9.4.2 Family members of Denied Special Identity Card for Expatriates, Article 138, paragraph 16, sub-paragraph γ (Law 4251/14) / Issuance

Duration: 1 year

  1. Certified photocopy of all pages of passports covering the last 3 years.
  2. Documents certifying cohabitation with the sponsor and 3 years of residence in the country.
  3. Electronic fee of 150 Euros (fee code 2107).
  4. Recent certificate of family status issued by foreign authorities, duly certified and translated, showing the family relationship.
  5. Proof that an application has been submitted to the respective insurance agency (IKA – Social Security Institution, OGA – Agricultural Insurance Organization, ΟΑΕΕ – Organization of Insurance for Self-Employed Professionals)to cover the costs of healthcare, medical expenses, and work-related accidents or Printout of insurance capacity issued by a public insurance agency (EFKA – National Social Security Entity) with the submission of: A.M.K.A. – Social Security Number, Α.Μ.Α. – Social Security Member Registration Number, A.F.M. – Taxpayer Identification Number
  6. Documents for the issuance of the card:
    • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format.
    • Card fee of 16 Euroscovering the printing of the separate document (electronic residence permit) (e-Paravolo, fee code 2119).
    • Official public document from the country of birth or origin, officially certified and translated, clearly stating the place of birth. The translation must also indicate the place of birth in Latin characters.(To be provided only if the passport does not show the place of birth in Latin characters)
