1. Certified photocopy of all pages of the passport or other travel document covering the validity period of the last residence permit.
2. Copy of a valid residence card.
3. Printout of insurance capacity (direct – indirect) issued by a public insurance agency (EFKA – National Social Security Entity) with the submission of: A.M.K.A. – Social Security Number, Α.Μ.Α. – Social Security Member Registration Number, A.F.M. – Taxpayer Identification Number
   OR Insurance policy by a private insurance company.
4. Certified copy of the tax clearance note or other evidence of financial resources.
5. Death certificate of the EU citizen in case the family members have resided in Greece for at least one year before his/her death,
   or Divorce Decree/Deed of dissolution of civil partnership, provided that the marriage/civil partnership lasted at least three years, one of which in Greece
   or Divorce Decree/Deed of dissolution of civil partnership, and court decision or written agreement of the spouses/partners on the custody of their minor children in case this has been entrusted to the spouse who is a third-country national,
   or Certified copy of the complaint submitted to a competent Greek authority in case of domestic violence,
   or Any other evidence indicating particularly difficult circumstances,
   or Written agreement between the spouses or court decision which regulates the right of contact of the spouse who is a third-country national with their minor child in Greece,
   or Death certificate or solemn declaration by the spouse/partner for the departure of the EU citizen from Greece and certificate of enrollment of the children in an educational i
6. Documents for the issuance of the card:
   • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format
   • Card fee of 16 Euroscovering the printing of the separate document (electronic residence permit) (e-Paravolo / fee code 2119).
   • Official public document of the country of birth or origin, officially certified and translated, clearly stating the place of birth. In the translation, the place of birth must be written in Latin charactersas well. (To be provided only if the place of birth is not indicated in Latin characters in the passport).
