1. Certified photocopy of a valid passport or other travel document recognized by our country.
2. Certificate from an institution or legal entity of public benefit, proving accommodation as well as the coverage of living expenses and medical care.
3. The Articles of association of the institution or Government Gazette (FEK) regarding the establishment and operation of the legal entity.
4. Decision of Appointment of the legal representative of the institution.
5. Recent certificate from a Greek State Hospital or private doctor, with the signature of the attending physician duly certified, or a decision issued by a health committee indicating the severity of the illness/accident.
6. Court decision appointing a guardian ad litem or a document issued by a public prosecutor ordering the placement of a minor in an institution or legal entity of public benefit.
7. Documents for the issuance of the card:
   • Four (4) recent, colored, passport-sized photos(40x60mm without a frame/high resolution/on a white background/neutral face expression, etc.), submitted also in digital form on an optical storage disk (CD) in JPEG 2000 format
   • Card fee of 16 Euros covering the printing of the separate document (electronic residence permit) (e-Paravolo / fee code 2119).
   • Official public document from the country of birth or origin, officially certified and translated, clearly stating the place of birth. The translation must also indicate the place of birth in Latin characters.(To be provided only if the passport does not show the place of birth in Latin characters)
