1. Two photographs.
2. Certified photocopy of passport or other travel document, if available.
3. Electronic fee of 150 Euros (fee code 2107).
4. Certified photocopy of the decision from the competent authority imposing restrictive conditions during the pre-trial stage.
5. Certification from the Police Department confirming compliance with the restrictive conditions.
6. Certification from the Police Department confirming that the restrictive conditions have not been lifted or modified (in case of exit prohibition only).
