1. Two photographs.
2. Certified photocopy of passport or other travel document, if available.
3. Electronic fee of 150 Euros (fee code 2107).
4. Certified photocopy of the council of misdemeanors’ resolution or the judicial decision, based on which conditional release has been granted.
5. Release order.
6. Certification from the Police Department confirming compliance with the restrictive conditions.
7. Certification from the Police Department confirming that the restrictive conditions have not been lifted or modified (in case of exit prohibition only).
